CREDIT REPORT AUTHORIZATION FORM

Authorization is hereby granted to American Financial Lending, LLC to obtain a consumer credit report through a credit reporting agency chosen by American Financial Lending, LLC. I understand and agree that American Financial Lending, LLC intends to use the consumer credit report for the purposes of evaluating my eligibility to obtain a loan.

I understand that this report will only be used by American Financial Lending (AFL) licensed representatives. This information will not be disclosed to anyone else by American Financial Lending (AFL) without my written consent.
